Summary : We report a case of impure nephrotic syndrome occurring during cysticercosis. Renal, biopsy showed membranous glomerulonephritis, indicating that there were immune complexes involved in reaction to this parasite. The patient had previously received prednisolone treatment without any success. Subsequently, treatment with praziquantel was followed by the disappearance of subcutaneus nodules with a decrease of proteinuria. This led to a remission of the nephrotic syndrome and, after an immediate post-therapeutic peak, lower serologic levels. Also, after an immediate post-therapeutic peak, creatinemia reached the proper value, which was lower than before the treatment, within twenty months of evolution. Because nephropathy complicated the cysticercosis and the antiparasitic treatment was favorable, research on this type of infection is interesting. Also, the serology of cysticercosis should be added to the list of antigen tests which could identify the cause of membranous glomerulonephritis.

Résumé : Un cas de syndrome néphrotiqtie impur au cours d’une cysticercose est décrit. Une biopsie rénale a montré une glomérulonéphrite extra-membraneuse, signant l’intervention de complexes immuns dans la réaction due à ce parasite. Un traitement par pratiquantel a provoqué la disparition des nodules sous-cutanés spécifiques de la cysticercose, une diminution progressive de la protéinurie entraînant une rémission du syndrome néphrotique et une baisse du titre des anticorps après un pic post-thérapeutique immédiat. Ce malade avait déjà reçu de la prednisolone, sans aucun effet favorable. La fonction rénale est restée stable car, malgré une ascension post-thérapeutique immédiate, la créatininémie est revenue à une valeur juste intérieure à celle avant traitement, après vingt mois d’évolution. Cette néphropathie compliquant une infestation par la cysticercose et d’évolution favorable après traitement antiparasitaire souligne l’intérêt de la recherche de ce type d’infestation. La sérologie de la cysticercose doit être ajoutée à la liste des antigènes responsables de glomérulonéphrite extra-membraneuse à rechercher.
